How do I make Check-boxes show up in RQM defect form?

I used the enumeration type field in a Task form for a project and it worked perfectly.  I tried the same in the defect form and only the field label is showing up.  Like some hidden field is covering the area where the check-boxes should be.

What exactly is an "RQM defect form". Is it the dialog you see when you try to create a new defect from within an RQM artifact?

Double check the Inline Work Item Editor of the Defect work item type in RTC. That's the editor that gets invoked within RQM.

Okay instead of creating a new enumeration field, I reused the field from the other project and it works perfectly.  I am not sure what was different.  Both were of the same type - Environment Found In (Enumeration List)
