Editing test script deletes previous steps

We are using RQM 4.0.1 with firefox 10.0.11ESR and IE 8/9. Issue: Create a test script with 3 steps. save it and close the window. Reopen the same test script and modify any one step and save again. Refresh the test script. Only the last modified test script step remains. Rest of the steps disappear. Please let me know if this is a known issue and any workarounds to fix this?
