RPE 1.3 with DNG 4.0.6 - Reports does not appears on DNG Report Wizard

The link you provided does not show a 'context' parameter in the MANIFEST.INF file - I think this important in making the template appear in the appropriate report wizard so if you have not included a Context that may be the problem. Typical values are:

Context: quickprintmodule

or

Context: view, module, collection

But it depends on what your template is for.

Also, after re-initialising the reporting service do you get the 'Initialization Complete Successfully' message?

If you are using separate web browser tabs or windows to initialise the service it may also be necessary to re-load any open page to see the newly initialised templates.

Yes, I received the successful message after re-initialising.

I tried this procedure in my VM instance and worked fine, but when I do the same procedure on server does not work.

You may check rrdg.log for additional information. The log file is location is <SERVER_INSTALL>\server\logs\rrdg.log.
