Unable to create a work item template

Client Version 4.0.5

Server Version 4.0.5 & 5.0.1

When I right click on the work item and click Create Work Item Template I am able to add the work item, but the Name and Description remain greyed out and I can't create the template.  I am an administrator and have full access and was able to create them on earlier versions.  Another admin on the project area saw the same thing on the same version client.

you may check with client workspace's .log and ccm.log in server at the timestamp and see if thereĀ are more clues there.
