What's causing duplication of the same artifact to appear in RRC?

Have tested this with various IE versions and Firefox, so not a browser issue.

Yes I would definitely contact Tech Support as we did not introduce the lock mechanism until V4.0.1

I would suggest contacting Tech Support to investigate.

Also which version of RRC are you using as we found a situation in an early V4.0.4 version with the artifact lock service (when editing) where two lock resources was created for the same resource, from different users

This is with 4.0.0.1, which we've been using for a while, but has only just been seen (or least reported).
