Is there any template and config file available to upload test cases along with requirements and precondition

Hi Team

We are using RQMExcelImporter plugin to upload our test cases in RQM. We have few templates available in sample folder  of the tool. But we need templates with below specifications:

Test case + Test Scripts(with steps) + Precondition + Requirement (Artifact ID to be added to Validate requirements column)

Please help to share the config file and excel template for uploading the test cases in RQM as early as possible

Configuration file will vary based on your excel data layout, but you can reference following sample file - 

Testcase and Testscript : Test Cases made from scripts.cfg
Requirement linking : TestPlan Requirement Collection Test Case and Requirements.cfg 
Testcase precondition : TestCases 001 with custom attributes.cfg
