The processing instruction target matching "[xX][mM][lL]" is not allowed error appears on trying to set permissions
Have upraded RTC to 5.0.2 from 4.0.1.
I noticed all set permissions for all roles had disappeared.
Also when I try to set permissions i get the following error:
The processing instruction target matching "[xX][mM][lL]" is not allowed.

That's not a good sign. It appears that the process configuration (stored in XML format) is corrupted. Similar posts on the internet all point to corrupted XML content, such as this one.
