How do i Found the URL of the change sets Link?

Hello All,
we have one requirement, In which I have to show the change sets Links in report which are present in the Link Tab of WI. my Problem is i Want to show change sets in same format which are displayed in the Link Types of WI format is Like this : Stream Name-creator-comment date .How can i get that ?
Another thing is after displaying Name of the Change sets Link, when i click on that Link it directly redirect to the Web UI, How can Find the Link of this Web UI Change-sets Links ?
