I have a user reporting that when they update Test Cases by removing links to Requirements the Requirements still reflect the link to the Test Case.

What can be done to make sure that when one is updated the change is reflected in both records (Test Case and Requirement)

The info center does note that the "synchronization between bidirectional links and the traceability between source and target artifacts are not guaranteed."  So I don't think there is a way to make sure they are always in sync.
Knowledge Center: Associating test cases with requirements

There was a feature introduced in RQM 4.0.1 to repair backlinks from RQM documented in the following link, but this is for a different use case.
