Report Design perspective in RTC 4.0.3

I have installed RTC v4.0.3 (server and client) and BIRT 2.6.2 r262; however, when I try to open the Report Design perspective.  When I go to Window > Open Perspective > Other, there is no option for Report Design.  Any suggestions would be appreciated.

Hi Jeremy,
We need a bit more information from you:
1. How did you install RTC client (using installation manager, from plain zip, on existing eclipse)
2. How did you install BIRT? Could you provide exact steps or the documentation you have used?
3. Please check <your-workspace>/.metadata/.log file for any errors. Do you have any errors there?

1.  RTC client was installed via Instalation Manager.

2.  To install BIRT, I went to Help > Install New Software.  From here, I installed the Business Intelligence, Reporting and Charting plugins from the following sites:

http://download.eclipse.org/releases/galileo

http://download.eclipse.org/releases/helios

And Charting and Reporting from:

http://download.eclipse.org/releases/ganymede

3.  There does not appear to be any errors in the log file.
