ERM diagram is created, saved, then disappears

User creates a diagram artifact in a folder then opens it in another Chrome browser tab.  He works on it, saves it, but when he tries to add that diagram artifact to a document artifact (not a module), he can't find it.  It no longer shows up in the folder and he can't find it based on the artifact ID.  It just seems to disappear.  Has anyone encountered this or is it a known bug?

I suggest opening IBM ticket for this issue. I encounter something similar but the reason was full database not web browser problem.
