Has anyone else had thousands of Work Item Queries!

Using RQM I raised a number of formal reviews (but no more than about 20). The next morning, I go to the Project Dashboard and I find 2639 Work Item Queries!

Most were "Already approved by me (0)" (but I had not approved anything, I was the person who invited one colleague to approved about 20 test cases).  I have seen this intermittently several times. Does anyone have an explanation?  Has anyone else seen this kind of issue?

Hi Mike,

Which version of RQM you are using and faced this problem.
Internally we have not seen this.
Please raise a PMR for this issue if it happens consistently.
