The arrow next to Expected Results has disappeared, how can this corrected?

screen shot - arrow missing next to Expected Results

The Test Script is not locked but the arrow to select insert step is missing for on user.  Is there a way to re-display this column?  RQM 3.0.1.3

User is Project Admin, changed to TestLead to see if there was issue with Role.  All other users see

Julie,

Is the user that is encountering this issue using the same Browser/Version as the users who don't see this issue?

Both are using FireFox ESR same version

This user had to show comments column to display the arrow.  Others do not.
