RTC 4.0.6 -> 5.0.1 Upgrade Info

I was wondering if this forum could help me out? We were planning to delay the upgrade of RTC 4.0.6 -> 5.0.1 until we were setup in the cloud –but- recent events concerning eclipse clients and Java version force us to consider doing this earlier. We not only have to upgrade RTC but upgrade MSSQL from 2008 to 2012 on Windows Server 2008 R2 OS. My question is has anyone on this forum done this. Is there a procedure on haw to do this? My current manager is looking for any guidance concerning this and a ballpark figure on how long this process will take. We need to plan an RTC outage time with Development/QA. Any insight concerning this would be fantastic!
