RM Artifacts are not updated in Review

I have a user that has created a review. In the review is one artifact. The artifact is a module. During the course of the review the artifact was updated. 
We have attempted using the Update Artifacts link as well as removing the artifact and adding it back. Either way we still end up with an older version of the artifact.
 If I view the artifact outside of the review I can see the updates. The review was updated to include the latest versions of the artifact, and from the review view it says it has been updated. However, if you open the artifact within the review, it is an older version.

Need some help figuring out why the artifact within the view is not being updated even though the Review says it has been.
This is on CLM 4.0.3.

I would attach some screenshots of the Review view and the Overview, but apparently I need more creds...
From the overview of the artifact (when looking at it outside of the Review) I see:
Modified On: Feb 20, 2014 1:52:19 PM

From the overview of the artifact (when looking at it from within the Review) I see: 
Modified On: Feb 12, 2014 10:33:33 AM

From the Review view itself I see:
Version: Feb 20, 2014 9:07 PM
Artifacts capture on: Feb 20, 2014 9:10:24 PM

Not sure whether or not this will meet your operating requirements, but you could also try using an informal review instead of a formal review for modules (temporarily), until you are able to upgrade to 4.0.6 for the fix that Hakan mentioned. Informal reviews should always use the latest version of the artifact under review, instead of reviewing a version of the artifact from a specific point in time from the past.
