Reproduce a sharepoint form in RTC

I have looked through the answers and could not find anything that looked similar.  We are trying to simplify and stream line our document handling process.  I have a new item form that has multiple sections all on one page, which is what we are currently using to fill out. - see example below.   Is this possible to do this in RTC?  We want to keep the forms in RTC that we use as similar to what our users are currently using.

There will also be multiple description sections

I believe you can create form similar to yours. Take a look at Work item editor presentation article.
