What is needed to get Approval Trackings to work?

I setup Approval Trackings but artifacts are not transitioning automatically based on approval state. I also granted the permissions to "trigger a workflow action" for all artifact types. What else is needed?

Please check that there are no conflicting preconditions & follow-up Actions (see http://pic.dhe.ibm.com/infocenter/clmhelp/v4r0m1/topic/com.ibm.rational.test.qm.doc/topics/t_ovw_def_preconditions.html) for those artifact types.
