RQM to RTC Defect creation with customized fields

Hi,

I have configured Defect work item in RTC. Now I want to populate Defect field directly from RQM.

I checked in RQM. When we create Defect after running Test Case/script , we get a page where we can set Defect Attributes. I would like to know how can I customize that Defect page in order to design it as per my  configured fields in RTC. I want to populate all those fields automatically from RQM in RTC.

e.g. Module is a field of Defect Work Item in RTC. I want to populate this field from RQM directly while creating Defect.
