this is currently not implemented. You could create a precondition that would check for this (see https://jazz.net/library/article/1000 ) or other examples such as https://rsjazz.wordpress.com/2012/07/31/rtc-update-parent-duration-estimation-and-effort-participant/ .

I personally think that the cost for doing it probably defies the value.

By "name", do you mean the "Summary" attribute?
If so, yes, you can have multiple work items with the same Summary string.
On the other hand, if by "name", you mean the integer ID (e.g. "Defect 1357"), then that integer is unique for a given repository.
