The report uses the result points rather than the overall result value.
Is it possible that the result of the overall execution of the TCER was set to Fail but none of the points were set to fail?

I'm not sure why they points didn't default.
I think, at the least, you are typically prompted/warned when the result is saved that the overall result is fail but no points were marked failed.
