At the top of the page it states:

Upgrading to version 4.0

This guide describes upgrading Rational Team Concert 3.0 or the server applications for Collaborative Lifecycle Management (CLM) version 3.0.1 to CLM version 4.0.

Note: Upgrading from version 2 to version 4 is a two-step process. You must first upgrade your server to version 3.0.1, then upgrade to version 4.0. For instructions on upgrading from version 2 to version 3.0.1, see the 3.0.1 information center.

The step wise document can be found in the interactive upgrade guide in the help. We created a workshop for going to CLM2011 because that was very complex.
