In Project Area|Process Configuration|Configuration Data|Work
Items|Approval Tracking you only define what should happen when an
approval changes it status.

What you are looking for is a precondition that would be configured for
the work item save operation. This precondition would check for a state
change and if the save causes a state change would check for the
approval. Such a precondition is not available out of the box.
