Defects created during execution nt listing when run a Query

Hi,

We found an issue when ran a query to fetch the defects based on a Test Case name , where in Defects which are created during Test Case execution are not getting listed.

When you open any TER records you will find all the Defects listed in the right hand side with Unique Defect ID.

But when running a query you will find only the one created normally.

I doubt some where we need to do some linking. I tried another query by name "All Defects" which is listing all the defects.

 I'm seeing the same thing. We are running RQM 4.0.1 with defect housed in RTC. But this widget just doesn't show any defect info. 
I'm wondering if it's to do with the data warehouse it uses in v4.
