When someone raises a defect they seem to get a lot of emails form RTC. How can we prevent this and just have emails for when the defect is raised, fixed and assigned to test?

Check the user profile. Some email options are there.

Take a look at this page:

It shows you what RTC functionality is provided by the Contributor
license (and the other license types).
There is a similar page for RQM and RRC.

Each user configures this for himself/herself. In the web client, in
the upper right hand tool bar, click on the down arrow next to your
name, and selects "view my profile and licenses", and select the "Mail
Configuration" tab.

I couldn't easily find this info in the on-line help, so submitted work
item 187710 to get this info added.
