My customer is experiencing a strange problem...
Some predefined queries are added into RTC project area. Their names are:

!myAlreadyApproved.name!
!myAlreadyRejected.name!
!openRequirements.name!

Any idea about how / why they could be added?

Have they enabled the integration to RRC recently? These sound like the sort of queries you might have in RRC.

No, they are not using RRC at this moment.
