Finding stories that keep moving

I am trying to figure out how to build a report that helps me see the track record of a team or a project with respect to stories continuing to slip from iteration to iteration. With a large project it is hard to keep tabs on stories that continue to not be complete from iteration to iteration. Has anyone tried to build a report that can identify these stories?

One way might be to look for stories that have tasks completed in different iterations. You may also be able to look at the history of a story to see how many different iterations it has been planned for.

Any one try this or have a better way?
