The Extensions Workshop https://jazz.net/library/article/1000 explains how this works and how to get to a working example as well.

No, as far as I can tell, there is no difference between application servers or databases used.

If you have custom plugins, you would need all the files like provision profile and features, to be able to deploy.

Enable Widget Extensions in DNG (softacus.com)
