How to upgrade RTC version from 4.0.5 to 6.0.4

We are planning to upgrade CLM version from 4.0.5 to 6.0.4. Please provide the necessary action and steps to achive the successfully upgrade.
Current Environment details:
1. Operating System: Suse linux 11sp4 (IBM Z system)
2. Database: IBM DB2 Enterprise Server edition 9.7 3. Application server: IBM Websphere 8.0.0.7 4. User registry: JTS internal user registry.
I knew, The upgrade need to be done in 2 steps - version 4.0.5 to 5.0 and then from version 5.0 to 6.0. But i want to know the exact version of 5(5.0.x) and 6(6.0.x) to the version upgrade.
Please help me for the same.
