workitem type Icon missing on work item when associating the work item

Current version of RTC 6.0.3

In the WEB UI when uploading a new change when I associate it with the work item the work item type icon is missing.

When I associate the work item in order to upload my new change and deliver to the stream, the icon is missing from the work item I associated with the change I am uploading.  Associating the work Item when uploading a change is the only time the work item icon is missing.

Any ideas?
