Understaing RTC- SCM change set version ID

Hello,

I am trying to understand the change set versions and observer there are changes set version is missing in some cases. below is the screen shot from File>Team>show history.
Could you please tell How to identify Version 4 and why it is missing in this case.


Also i some case we have same version ID  for change set when we merge.

In addition, another possible cause of missing version ID is someone else check-in the same file from another workspace and has not delivered the change set. As the version ID is generated for every check-in in sequence.
