Is there an example for Contributing Attribute Presentations for Jazz 5 or 6

There have been several changes in server side APIs, including name changes in dependent bundles, extension points etc, these changes render the Contributing Attribute Presentations example (Larry Smith's 2011 Wiki entry) irrelevant.  To make matters worse, the attached source code does not match the example.

I have been trying for days to register the "starred kind" as a presentation kind.  I have successfully deployed Advisers and Participants.  So I'm not having a problem with the packaging and deployment of the eclipse-plugin.

Ultimately, what I'm trying to do is this;

1) Enable and disable controls and dropdowns.
2) Clear dropdown value when the value of another dropdown changes

Any help would be appreciated.
