Why isn't user able to add comments/alter change sets owned by other users?

User lost ability to alter change sets owned by other users.  A rule we try to enforce with restrictions change sets is that there must be a comment.  This user used to be able to do this without any problems for others folks change sets.  After a RTC crash, the user lost the ability to alter change sets.  He now receives the error:

Problem
Permission denied during "Source Control Operation".

Permission denied. Only the owner of the Change Set can modify it. Current owner is userA and you are logged in as userB

We are currently using RTC 4.0.6.  The user has the proper role/permissions to save change set links and comments, modify comment, and modify work item links.  Is there anything that else needs changing?
