There are many questions to ask for this kind of issue.

1) RQM version and if it was upgraded from a lower version?
2) whether the same issue takes place in new test case or existing ones?
3) if the issue only happens to specific test case or not
4) if other test artifacts are having the same issue: test plan, test script etc?
5) when you see the issue, check with qm.log file at the specific time and see what is the error messages there?
and so on.

Brian reported the same issue some time ago and resolved it by following the instructions in the accepted answer. You can also give it a try.

The steps are basically for re-deploying the application. For WAS, what you need to do is as follows.
1. Uninstall the application from the WAS admin console.
2. Stop WAS.
3. Clear the WAS temporary folders (temp and wstemp under the WAS profile).
4. Start WAS.
5. Install the application using the WAS admin console.
