RQM excel Importer: how to import date fields into RQM using Excel addin 5.0.2

Can anyone please provide me steps to import the datefields into RQM using excel add. please provide me the template and config file.

Please see the sample "Testplan_Testcase_Testscript_TER_Environment_Iteration_AllInOne.xls" that comes packaged with the utility, it has a number of Date/Time fields included
