IBM Rational Team Unifying Platform and Jazz are both similiar as they are used to unify a team by providing common access to development assets, communication alerts, and workflow processes.

Jazz supports the following software:
Rational Focal Point for Project Management, Rational Team Concert, Rational Quality Manager, and Rational Requirements Composer.

IBM Rational Team Unifying Platform supports the following software:
Rational ClearCase LT, Rational ClearQuest, Rational MethodComposer, Rational ProjectConsole, Rational TestManager.

"IBM Rational Team Unifying Platform" is product sold by IBM/Rational,
which is a set of software tools, in particular, as Stephanie said:
ClearCase LT, ClearQuest, MethodComposer, ProjectConsole, and
TestManager (see http://www.ibm.com/software/awdtools/team/).

"Jazz" (only the first letter is in upper case, because it is not an
acronym for anything :-) is not a product, but rather a team software
platform upon which products can be built. So RTC (Rational Team
Concert) is one such product. RQM (Rational Quality Manager) and RRC
(Rational Requirements Composer) are two other products. Note that Jazz
is used not just in new products, but is also used to integrate and
enhance existing products/repositories (such as ClearQuest and ClearCase).
