Why would previous TSER Not Appear After Upgrading From 4.0.3 to 4.0.5?

I'm wondering if anyone else has ever run into a problem associating TSER to a Test Plan after an upgrade from 4.0.3 to 4.0.5?    If I create a TSER from inside the test plan, I cannot choose the Test Plan:

However, if I run from the test suite, I can see the various options:

For this particular suite, there were two execution records created in 4.0.3.   A brand new test suite can be run just fine through the test plan.

Hi James,

Were the old TSERs associated with same Test Plan or a different one?
I see similar behavior  in 4.0.6 if the Test Suite is added to  two different Test Plans. If there is a TSER already on one Test Plan, you cannot create a new one for second Test plan.

Vidya, yes that is the problem.   I see it was identified in 5, but it is present in 4.0.5 as well.
