RTC upgrade stuck on Finalize Application Setup

I try to upgrade an RTC server from 3.0 to 4.0.4. When going to https//....../jet/setup evereything works fine, applications are registered etc.
When reaching the step 'Finalize Application' for the ccm application the [Finalize Application Setup] button remains greyed out and I get stuck. I checked the server and consequently the ccm application can not communicate with jts, since oauth is not correctly configured, users are not synchronized etc.
Shutting down the server and running through https://..../jts/setup again, I can quickly skip with the [next]buton to the same page.
The jazz.log file contains the following errors:
as well as subsequent authorization errors.
I hope someone out there has some idea on how to go further.

Hi Erik,
What are the instructions you were trying to run upgrade? I have reviewed following ones: http://pic.dhe.ibm.com/infocenter/clmhelp/v4r0m3/topic/com.ibm.jazz.install.doc/topics/roadmap_clm_upgrade.html and I didn't find any mention to rerun setup again.
Let me know if I have missed something.

In the Lifecycle Project Administration (LPA) application, you can create and manage project areas and establish associations between them. The LPA application was first introduced in CLM version 3.0.1. So if you are upgrading from Rational Team Concert 3.0, you must configure and register the LPA application with Jazz Team Server.
