How to upgrade RTC 3.0 and RQM 2.0.1 to CLM 4.0.3?

 Hello Friends: I have the following comment: I have a server currently RTC 3.0 and RQM 2.0.1 both running on a server with WAS and pointing to another server with DB2.
Now we have a better architecture, this is:
1 New Server with JTS and RM 4.0.3,
1 New Server with CCM 4.0.3
1 New Server with QM 4.0.3
1 New Server with DB2 9.7

How should I proceed with the migration of data from old versions of RTC and QM to version 4.0.3? wonder that updates discuss the procedure in the same server ...

All the help, guidance and action steps is welcome
