RQM Adapter cannot register

I cannot get this issue resolved when trying to connect to RQM using the adapter.  I have searched all the similar threads and verified the settings again and again but no luck. These are the errors from the error log:

RPTX2018W Communication error with server.java.net.ProtocolException: PUT[405] : <html><head><title>Apache Tomcat/7.0.25 - Error report</title><style><!--H1

RPTX2025E Error occurred while registering the adapter.com.ibm.rqm.adapter.library.data.AdapterException: Failed to send data

Any suggestion?

Namit,

Can you provide some additional information
The version of RQM?
What type of adapter (i.e. RFT, RPT, Command Line..ect) and version?
The URL you are specifying to connect the the RQM server?
..and if possible, the adapter log

Thanks Ara for responding. RQM version is 4.0.0.1. I am currently testing it with RPT and RPT is at ver 8.3.

The url is https://servername:9443/qm/admin

Let me know if you were looking for some other log but here is the adapter log from my machine where RPT is installed:

Display unavailable, no RPT HTML reports will be attached to RQM execution results.
Communication error with server.
Error occurred while registering the adapter.
Adapter is successfully connected to the server.
Adapter successfully reconnected to the server.
Communication error with server.
Adapter successfully reconnected to the server.
Communication error with server.
The RQM Adapter has stopped.

Hi Namit,

Please provide the url upto - https://servername:9443/qm
Do not include /admin in the url.
