Migrating Work Items into RTC from another SCM tool

I can get change requests from one project in another SCM tool (StarTeam) into a comma delimited file. What would I need to do to get them into RTC? I would image I would have to create the custom fields in RTC and then I would somehow have to map the fields. Would this have to be done programmaticly?
