RQM 3.0.1.1 hangs when add TERs to test plan

Summary
RQM hangs "Saving Test plan" when creating multiple Execution Records

Problem
This is from IBM Business Process Manager product who uses RQM for our test management.
When creating execution records for multiple test-cases, this will cause RQM to hang by looping in the 'Saving Test Plan' (see screenshots for steps and results).
Server has been rebooted to clear the cache but the same problem occurs. This occurs with different user IDs
Note that once the test plan hangs when saving the test-cases, then user has to wait for 30 mins to create the ER for the same test-case. Logging out / deleting the cache doesn't help.



Environment
AIX6.1 using WAS 7.0.0.19/DB2V9.5 FP7
RQM 3.0.1.1
