Add "Search topic title only" to forum search

Currently the options for search are:

I would like to add:

Search topic title onlyThis would greatly improve the relevance of posts returned by searches. I have created a mockup below. While you are it, you can fix the word wrapping that was likely unintentional (and occurs in both Firefox and IE regardless of browser window size).
