Usage model - Peer Review

I'm trying to do a Peer Review (code syntax, etc..) for our work items.

The main problem we see, is that if the work item being delivered is not good, then, not approved, and so on, creates several changesets all related to the same work item, containing changes in the same files, and its hard to understand why was the peer review rejected
- i.e. which files/changesets had the problems and should be fixed.

Any ideas? recommended usages?
