Requirements 001 Template Has anyone got a working version

Just downloaded the RQM Excel Importer, however the Requirements 001 template which is contained in the sample files, has incorrect mapping.

Does anyone have a modified version that is working or come across this issue before.

the sample in the RQM excel Importer 4.0 GA works fine.
