Migrating Plans and work items between Jazz servers.

I currently have 2 servers running jazz and I would like to move all the information from one to the other, so I can stop using the old one.
Is there a good way to migrate all the data from one server to the other?

We need to migrate Work Items, Plans etc. Source control is not used so we don't need to migrate builds or sources.
