Ideally, When a Reviewer is added for a Testcase. Email notification should be sent to the reviewer. In RQM where we can enable mail notificationsent for test case review.
Test artifact review/approval notification uses the legacy work item notification hence you would need to go to user(this is configured per user base)'s profile and then configure from there:
