Export defined informations from ETM Test case to MS Word (Test report template)

I´m searching a solution to export defined informations from a test case in a separat (html file) or MS Word.

I want to export from a test case for example :

- linked requirements (ID and text)

- test case ID

- test case version

- Test description

- pre-condition

- test case design

- expected results

- post condition

- test script steps

- attachments
