Please help me on how to find UUID for baseline and stream.

You can get this information via QM's Reportable REST API: https://jazz.net/wiki/bin/view/Main/RqmApi#Stream_Support

Also, from QM's Configuration management admin page, hover over a stream, or a baseline, you'll see the artifact's url link. The UUID of the artifact is embedded in the link.
