Icons not showing up on RTC query results or reports

We have a custom attribute call RGY based off an enumeration with the same name (RGY is Red, Yellow, Green) with a Harvey ball as the icon. The name is showing up in a query but not the icon. Anybody know why? The icons seem to show up on other non system attributes. Not sure how to troubleshoot this.

Just to clarify, when you say it does not show up, do you see an empty square (placeholder for the icon), or no square at all?
