Does anyone know what this err message means? why cant' I attach a doc to defect? how to fix it?

[We tried to upgrade to 6.0.1 but the upgrade failed, so we rolled back to 5.0.2, when we went back to 5.0.2 this issue came up].

The message is

Single row operation "{0}" on item handle "{1}" failed.
which means little actually. More information will be needed to fully understand the issue. I suggest you contact Support so that the log files can be sent in for further investigation.
